2008/2009 Updated Policies for Dispensed Medication including Controlled Substances

Effective in 2009, the Arizona State Board of Pharmacy is requiring all veterinarians to report the following information from each pet owner that is dispensed controlled prescription medication:
- Name
- Address
- Gender
- Date of Birth
- Telephone Number
- Method of Payment
Our front office staff will be asking for a form of photo identification to verify this information to keep on file. Please understand that we adhere to strict privacy guidelines and this information will never beshared with anyone other than the Arizona State Board of Pharmacy. Also, controlled substances can only be dispensed to the person(s) whose name and date of birth is on the account. If you would like to update your account information, please call us at 602-279-7366. We cannot dispense controlled substances to anyone under 18 years of age. We appreciate your cooperation and understanding in this matter.
The list of our dispensed medications that we require photo identification for is as follows:
Butorphanol (Torbutrol, Torb. Liquid mix)
Hydrocodone (Tussigon)
Buprenorphine (Buprenex) - for cats
Fentanyl skin patches
Phenobarbital
Diazepam (Valium)
Tramadol

* Annual exams are required for all prescriptions to be filled and refilled; this includes prescription diets. Not only is this simply practicing good medicine, but is also a state pharmaceutical regulation we are required to follow.
* Certain medications require blood work to be done every six months. This is for the safety of your pet, as the blood work can screen for any issues with internal organs. This helps to check for any internal side effects of the medicine and help to adjust dosages if needed. Please schedule your blood work and refills accordingly. Usually, blood work canbe scheduled as a 15 minute appointment with a Technician, and the doctor will call you to review the results.

Refills: Please allow 48 hours for all prescriptions to be refilled. Pharmacy regulations require that all dispensed prescriptions are approved by a doctor, filled, documented and double-checked, which can take up to 48 hours depending on our schedule.
Returns: Federal Pharmacy Regulations prohibit the return of any prescription medication.
